Step 2 — Pay the fee: Take page 4 (bank challan) to any MCB Bank branch and deposit Rs. 1,000. Ask the bank to put its stamp and signature on the challan.

Step 3 — Fill the form: Write in CAPITAL letters. Do not leave any box empty. Tick only one post. Paste a photo on page 1 and staple a photo on page 2. If someone helps you fill it, check your CNIC number and date of birth yourself.
Step 4 — Put the papers in the envelope (see list below).
Step 5 — Send by post: Send the A4 envelope (do not fold the papers) by post or courier to the address below. Write the post name on the envelope, e.g. "Naib Qasid — NCSW". Hand delivery is not allowed.
